Additional Website Privacy Information
Website supplement dated October 15, 2026. The Communication Privacy Policy above retains its original effective date of January 1, 2025. This supplement explains the website features and information involved in online inquiries; it does not replace our HIPAA Notice of Privacy Practices or provide a HIPAA authorization.
Information submitted through website forms
Our contact and consultation forms collect contact details such as your name, email address, and phone number. Depending on the form, we also receive the services or conditions you are interested in, appointment preferences, and any symptoms, diagnosis, pain levels, duration of symptoms, or other information you choose to describe. Please provide only what is needed for your inquiry. Do not submit Social Security numbers, payment card information, medical records, or sensitive documents through a general website form.
If you choose to provide them in an inquiry, survey, or offer response, we may also receive your job title, postcode or other demographic information, preferences, interests, survey responses, and other information relevant to your request. These are not all required fields on the current forms.
Website forms are hosted by LeadConnector, which receives information submitted through its embedded forms. We forward standard campaign attribution fields (utm_source, utm_medium, utm_campaign, utm_term, and utm_content), when present, to the hosted form to identify the source of an inquiry. Please do not place personal or health information in campaign links or URL parameters.
Website operation and uses
Website hosting and embedded services may process technical information such as your IP address, browser and device information, referring URL, page requests, and usage information. We use inquiry information for internal record keeping, responding to questions, scheduling and coordinating services, and understanding and improving the website and our services.
Where you have chosen to receive non-essential communications, we may use your contact details for service news, offers, surveys, or feedback requests by email, phone, or mail. Preferences and voluntarily provided feedback may help us tailor information to your interests. You may withdraw your communication preferences by contacting us. Merely visiting the website or reading this policy does not enroll you in SMS or authorize marketing use of protected health information.
Cookies, storage, and embedded content
Cookies are small files stored by your browser; similar technologies can store preferences or identifiers. WordPress uses cookies for functions such as login sessions and user preferences. Hosted forms, embedded YouTube videos, and ReputationHub review widgets may process technical information or use cookies or similar storage under their own policies when loaded or used. If analytics are enabled, they may use page-usage information to help us understand which content is useful.
The hosted contact and consultation forms include Meta Pixel and HYROS tracking scripts for advertising attribution and measurement. These services may process browser or device identifiers, page information, and interaction events under their own policies. See Meta’s privacy policy and HYROS privacy information. Contact our office by phone if you prefer not to use an embedded form. Do not include health information in public comments, tracking links, or campaign parameters.
Cookies do not by themselves give us access to the contents of your computer. You can accept, block, or delete cookies through your browser settings and use available third-party privacy controls. Blocking cookies may prevent some website or embedded features from working. Browser cookie preferences do not necessarily prevent every third-party network request. Contact us for another way to make an inquiry if you prefer not to use embedded services.
Embedded website services are not a secure patient portal. Please call us to discuss an appropriate way to send sensitive health information.
Third parties and information protection
The no-sale and no-distribution-for-third-party-marketing commitments in the Communication Privacy Policy above also apply to information submitted through this website. Mobile phone numbers, SMS opt-in information, and consent records are not sold, rented, or shared with third parties or affiliates for their own marketing or promotional purposes. Service providers may process information on our behalf to operate forms, communications, hosting, and related services, subject to applicable obligations. Information may also be disclosed when required by law.
Our website contains links and embeds operated by other organizations. Their privacy notices and terms apply to information they collect directly. A link does not mean we control or endorse the other website, and our policy cannot guarantee how an independent site protects information. Review their notices before sharing information: LeadConnector/HighLevel privacy information, Google and YouTube privacy information, and the privacy information supplied by a review or scheduling service.
We use reasonable physical, electronic, and administrative safeguards. No internet transmission or storage method is completely secure. General forms, ordinary email, and SMS should not be assumed to be secure patient portals. Contact us for an appropriate method to communicate sensitive health information. Records are retained as needed for the purposes described, applicable clinical record requirements, and other legal obligations; this policy does not promise immediate deletion of medical records.
Your choices and personal information requests
Review the choices presented in each form before submitting. You may decline non-essential marketing where that choice is offered, unsubscribe from email communications using their instructions, or contact us to change your preferences. If you previously agreed to marketing, you may change your mind. Reply STOP to stop SMS; reply HELP or OPTIONS for assistance. Message frequency varies and message and data rates may apply. Choosing not to receive marketing messages is not a condition of purchasing care.
You may contact us to request access to personal information we hold about you, ask for correction of inaccurate or incomplete information, or request deletion where applicable law allows. We may verify your identity and explain any legal or record-retention limits. Requests concerning medical records and health-information rights are addressed by our HIPAA notice. We do not promise that all categories of information can be deleted.
If comments are enabled on an article, submitted comment fields, IP address, and browser information may be used to detect spam. A profile image may be supplied by Gravatar, subject to Automattic’s privacy policy; approved comments can be publicly visible. Account holders can request information about their profile data. Do not post health information in a public comment or upload images containing sensitive information or embedded location data.
Policy updates and contact
We may update this website supplement by publishing a revised version on this page with its revision date. Please review it periodically. Changes to the HIPAA notice are governed by that notice.
